Determine if there is a plan, means, and opportunity
In addition, consider whether the patient has a plan and the means or opportunity for suicide. Patients suspected of having suicidal potential should be asked:

•  Whether they have an actual plan for self-harm

•  If they have a firearm or other lethal weapon in the home

•  If the patient's plan is credible in terms of opportunity and lethality
