Age: 55
Marital Status: married
Children: 2
Substance Use: drinks occasionally, smokes cannabis 1x per month, is willing to stop alcohol
Other Drug Use: none
Contracted Hepatitis C: experimental drug use
Candidate for Treatment: elevated LFTs, HCV RNA greater than 850,000 iu/ml liver biopsy grade 2, stage 4
Patient's Description of Mood: a little tired or discouraged, more up-beat than really down

Possible Indications of Depression in the Medical History:
  substance abuse problems (cannabis abuse?)
  sleep disturbances
  sexual dysfunction
  loss of social supports and/or isolation
  difficulty concentrating and poor work or school performance
  absenteeism
  multiple, vague physical complaints
  history of mental illness
  family history of mental illness

Possible Indicators in the Physical Exam:
  weight gain or weight loss
  paucity of facial expression or sad looking
  poor eye contact
  poor grooming or unkempt appearance, poor personal hygiene
