Once the patient understands the reasons why he or she should fill out the form and how the information will be used, you need to explain how to fill it out.

Which of the following paragraphs best incorporates all of the skill elements around "What am I supposed to do?"?

A) This form should only take you a few minutes to fill out. Just read each question and choose the answer that fits you best. Don't guess, just go with your first thought. You can fill out the form here in the waiting room with the other patients.

B) Don't worry. This is an easy questionnaire to fill out. Just read each question and circle the sentence that best describes how you feel. Don't guess, just give your first reaction. Feel free to ask me any questions.

C) This simple form should only take you a few minutes to complete. Just read each question and choose the one that you think describes you best. Don't think too much about the questions and don't guess, just pick your first thought. If there is something you don't understand, let me know. I am going to give you a few minutes to fill out the form in private.

D) Fill the form out. Do it as quickly as you can. Don't ask questions because that would change your answers. You have exactly 2 minutes to complete the form. If you are not sure, make your best guess on an answer. I will get you when my timer rings and indicates the 2 minutes are up.
